Community Care Management Services

RCJL recognizes that moving out of your home is not always the only option. We are committed to offering a wide variety of resources with elder care needs and since 1976 RCJL has met this need through our Community Care Management Services. In keeping with the RCJL goal of enhancing quality of life, Care Management Services can assist you and your family with assessments, consultations, geriatric counseling, home health services and locally for families who require out-of-area care. Rehabilitation

Residents who are in need of rehabilitation utilize our onsite center where highly trained and licensed staff are available to provide the services they require to reach their optimal level of strength and function. Services include physical, occupational, and speech therapy. All residents who move into RCJL are provided with an initial therapy evaluation. Contact Rehabilitation

Social Services

Under the direction of our Director of Social Services, the social service staff provides individual and group supportive services and assistance. Adjusting to new surroundings, new challenges in creating friendships, the feelings of loss and the fear of the future, are some of the issues residents express which social service staff is here to address.

Each resident brings unique needs and interests to RCJL. Upon admission, the social worker will meet with resident and family in order to identify, evaluate and plan for psychosocial needs. This may include obtaining outside services such as financial, legal, mental health and other community resources. We are often called upon to assist with formulating an Advanced Health Care Directive or work with the Medicare, private insurance, and Medi-Cal systems. In addition, regular Care Plan Conferences are held to assist each resident to maintain the highest level of physical, mental and psychosocial well-being. Contact Social Services
